Please register early. This deadline is firm.

Whether you are a new or returning Polarity Center student, this completed application is required for our records for any Level class.

You can apply and pay your deposit two ways:

Complete the form below and hit the “submit” button at the bottom. All fields are required. Pay using the PayPal button, selecting the Level for which you are registering.
Download the form, print it, fill it out and mail it with a check (Level I – $65, Level III – $275) to:

Registration fee and refunds
Please send a non-refundable $65 registration fee made out to the Polarity Center either by PayPal or by check. If your application is denied, the school will refund $35 of the registration fee. All fees paid by the applicant will be refunded if requested within three (3) business days.

Level I applicants: Please send us a recent photograph of yourself.

The Polarity Center keeps all records confidential.
