Applications are due by Jan. 15, 2012 for spring 2012 classes.

Please register early. This deadline is firm.

Whether you are a new or returning Polarity Center student, this completed application is required for our records for any Level class.

You can apply and pay your deposit two ways:

Registration fee and refunds
Please send a non-refundable $65 registration fee made out to the Polarity Center either by PayPal or by check at P.O. Box 345, 2531 Jackson Rd., Ann Arbor, MI 48103. If your application is denied, the school will refund $35 of the registration fee. All fees paid by the applicant will be refunded if requested within three (3) business days.

Level I applicants: Please send us a recent photograph of yourself.

The Polarity Center keeps all records confidential.
